The Full Story

STS-107 was a sixteen-day science mission, the longest Shuttle flight of its kind, packed with more than eighty experiments spanning biology, physics, and materials science. Its seven-person crew included Commander Rick Husband, Mission Specialist Kalpana Chawla, who had become the first Indian-born woman in space on an earlier flight, and Payload Specialist Ilan Ramon, the first Israeli astronaut to fly in space. What none of them knew was that the mission had already suffered a critical, undetected injury during launch.

Eighty-two seconds after Columbia lifted off on January 16, 2003, a piece of insulating foam broke away from the external fuel tank and struck the leading edge of the orbiter's left wing at high speed. Engineers reviewing launch footage noticed the strike and raised concerns, but NASA management determined, based on incomplete analysis, that the foam strike did not pose a safety-of-flight issue, and the crew was never informed of the full extent of the concern.

For sixteen days, Columbia's crew conducted their science mission unaware that the foam strike had cracked the wing's thermal protection, opening a breach that would prove catastrophic during reentry. On February 1, as Columbia descended toward a landing at Kennedy Space Center, superheated atmospheric gases entered through the damaged wing, progressively destroying the internal structure from within. Columbia broke apart over Texas and Louisiana, killing all seven astronauts, just sixteen minutes before its scheduled landing.

The Columbia Accident Investigation Board determined the technical cause was the foam strike, but like the Rogers Commission after Challenger seventeen years earlier, it also identified deep organizational failures at NASA, a culture that had normalized foam debris strikes as an acceptable risk rather than treating them as a serious safety concern. The Space Shuttle program was grounded for more than two years, and NASA implemented sweeping changes to its safety culture, debris monitoring, and on-orbit inspection procedures before flights resumed.
